If you change your name, your address, your telephone number, your cellular number, or your e-mail address, then The Office of Student Loans needs you new information.

UMC Institutional Student Loans Annual Certification of Deferment Status form

A deferment request based upon your employment should be filed no earlier than the deferment end date but no later than fifteen days after the deferment end date.

If your deferment end date is July 1st, then you should file your request after July 1st but before July 15th.

If you need to request for your UMC Institutional Student Loan to be placed in a deferment status, then complete and submit this form.

Some Institutional Loans have a maximum number of deferments; check your promissory note for your limit.

Do not use this form if your UMC Instituitional loan does not have a deferment provision.

Do not use this form for Federal Campus-Based Student Loans.

A deferment request based upon your employment should be filed no earlier than the deferment end date but no later than fifteen days after the deferment end date.

If you received a service scholarship, then this form must be completed and submitted at the beginning of each year of service until your service requirement is completed.

The Health Care Professions Student Loan is NOT a service scholarship.

A cancellation request based upon your employment should be filed no earlier than the deferment end date but no later than fifteen days after the deferment end date.

If your deferment end date is December 1st, then you should file your request after December 1st but before December 15th.

If you received The J. R. Scribner, JR Student Loan, then this form must be completed and submitted at the end of each year of service until your service requirement is completed.

Student Loan Entrance Data Form

The Student Loan Entrance Data form must be completed by all students that will be receiving funds from any Federal Campus-Based Student Loan Program or any UMC Institutional Student Loan Program.  You will be mailed this form with your student loan paperwork from The Office of Student Loans.

Federal Perkins Student Loan Deferment Request for EMPLOYMENT

This form is not for a resident or intership deferment.

A deferment request based upon your employment should be filed no earlier than the deferment end date but no later than fifteen days after the deferment end date.

If your deferment end date is March 1st, then you should file your request after March 1st but before March 15th.

Use this form if your employment will defer your Federal Perkins Student Loan payments.

You will complete and submit this form during the last month of your Grace Period.

After your first year of employment, you will complete and submit the Federal Perkins Student Loan Cancellation Request Form.

Medical and Dental students do not use this form.

Federal Heath Professions Student Loan and Federal Perkins Student Loan Deferment Request for DENTAL students

Only DENTAL students use this form for their Federal Health Professions Student Loans or Federal LDS or Federal Perkins Student Loans.

A dental student must be enrolled in a residency or an intership program to use this form.

Only MEDICAL students use this form for their Federal Health Professions Student Loans or Federal LDS.

You must be enrolled in a residency or an intership program to use this form.

There is not a residency deferment or intership deferment for the Federal Perkins Student Loan for medical students.  To request a Perkins deferment, use the Request for Economic Hardship or Forbearance form.

Federal Perkins Student Loan Cancellation Request due to Employment

A cancellation request based upon your employment should be filed no earlier than the deferment end date but no later than fifteen days after the deferment end date.

If your deferment end date is March 1st, then you should file your request after March 1st but before March 15th.

Use this form if your employment will cancel your Federal Perkins Student Loan.

After your FIRST year of employment, this form must be completed and submitted at the beginning of each year of service until your service requirement is completed.  Remember to check the Deferment for Pre-Cancellation Services Box.

Medical and Dental students do not use this form.

Economic Hardship Deferment Request or Forbearance Request For Federal Campus-Based Loans and Institutional Loans Instructions

An Economic Hardship Request or Forbearance Request for your campus-based loans and/or for your UMC institutional loans should not be filed until thirty days (30) prior to your payment due date.

If your payment due date is June 1st, then you should file your request after May 1st but before May 15th.

If your UMC Federal Campus-Based Student Loan and/or UMC Institutional Student Loan does not qualify for an automatic deferment or your automatic deferments have been exhausted, then you may qualify forn an Economic Hardship Deferment or a Forebearance Deferment.

All UMC Instititional Student Loans do not qualify for Economic Hardship or Forebearance status.  See the UMC Institutional Student Loan List for information about your institutional student loan.
